There are three layers, and only one of them gets cleaned
A carpeted floor is not one thing. There is the carpet you can see, the underlay beneath it — usually foam or rubber, exactly the sort of material that soaks up liquid and holds it — and then the floor itself, timber or concrete slab.
A spill of any size stays in the carpet. A larger volume of liquid, and pet urine in particular, does not: it passes through the carpet backing and into the underlay, where it spreads sideways under the carpet and dries into the foam. From that point the visible mark on the carpet is the smallest part of what is actually there.
That is why a room can be cleaned, smell fine for a few days, and then go off again — especially on a warm or humid day. The odour compounds are in the underlay, and warmth and moisture reactivate them.
